JGS is looking for a Speech-Language Pathologist to join its Home Care team. The SLP evaluates, treats and rehabilitates clients by providing them with necessary therapeutic intervention. The SLP maintains pertinent documentation, carries out appropriate treatment plans and contributes to discharge planning.

• Completes screens (as scheduled)

• Initiates and follows through with restorative programs as appropriate
• Attends patient care meetings (as assigned)and actively participates
• Follows through with discharge planning, including: equipment ordering, family teaching, home safety recommendations and discharge documentation (as appropriate)

In addition to family programs, Home City Families is an information and resource hub for families. The Home City Families Early Childhood Program Specialist helps to provide comprehensive support for children and their families through ongoing educational programming, family support and connecting families to a broad range of resources throughout the city of Springfield.

DUTIES:

Facilitate parent/child educational playgroups with a focus on literacy and language supports for Springfield families targeting families with children birth through 5 years old
Assist with creating developmentally appropriate playgroup structure, activities and curriculum
Assist with developing and promoting programming for families with young children
Provide a variety of family education and support activities that strengthen knowledge of child development and provide families support in the overall development of their child
Participate in community outreach to ensure the community has knowledge of what we have to offer
Support families with young children with transtions to school and/or programs
Provide families with resources and referrals to community agengies to best meet their needs
Provide outreach visits to new parents throughout Springfield to connect them to community resources and offer a connection of ongoing support to their new family
Assist families in the completion and follow-up of the Ages and Stages Questionnaire (ASQ) screening tool to enhance their understanding of child development and to link them with community-based resources
Maintain accurate data for all programs for State reporting requirements
Performs other duties as assigned by Project Manager

QUALIFICATIONS:

Associated degree in early childhood education or related field
Requires minimum 2 years of experience in the early education field
Strong knowledge of family dynamics, early childhood development, and early literacy
Spanish bilingual skills (speaking, reading, writing and listening) highly desirable
Communicate effectively with families in a friendly and supportive manner
Knowledge of community resources available throughout Springfield
Ability to facilitate group activities with diverse families
Outstanding organizational, communication and interpersonal skills

Full Day/Full Year position $41,686
